Western Queensland Health Care Home

Strengthening general practice and service provider capability and innovation building a contemporary patient centred primary health care strategy.

The Western Queensland Health Care Home (WQ HCH) program has been replaced by the Primary Health Care (PHC) Practice Support Program, and has refreshed the foundations pieces of General Practice.

The aim of our Practice Support Team is to enhance the data integrity and recording of clinical data sets to provide clear visibility and transparency of data collected at general practice level. This data will then inform practices of any follow-up required to ensure quality care is provided at the right time and place, and the practices operate efficiently and effectively.

The new suite of tools have been developed to reflect the requirements for all staff of general practices to enhance the data visibility through the WQPHN PHC Dashboard.

The tools provided support Medicare requirements, RACGP 5th edition requirements, Continuous Quality Improvement (CQI) activities, and MyMedicare requirements in line with Practice Incentive and Workforce Incentive payments.

The new refresh will enable the data transparency to be addressed through education, training and outcome-based evidence. The uplift and cleansing of data will assist the strategic direction requirements of all PHC services to provide the patient-centred care outcomes required for good governance, and provide outcome-based evidence of improvement through the use of the tools and data platforms, with a focus on CQI initiatives.

The benefit of the new PHC Practice Support Program will be to provide transparency and visibility of the data, with a focus on data cleansing, Continuous Quality Improvement and identification of care required to provide holistic preventative primary health care services to the rural and remote communities we support.

The PHC Dashboard is sent via a secure link to each provider monthly with trend data to map the improvement of data quality of each site.
